To submit a request for an agenda item to be considered for placement on a Board or Committee Meeting AGENDA, please complete and submit the following form, including all supporting documents, on or before the 20th of the month to be considered for meetings in the following month.
Please prepare text documents in MS Word format. Use the exact language which you wish to appear on the agenda in final form, including names, titles and email addresses of addressees and CC recipients if applicable. Once we have reviewed all requests you will receive a confirmation.
